  • Trained Dementia Champions and offered Dementia Friends Information Sessions
  • Provided adult education workshop for congregation, including caregivers and pastoral staff
  • Collected and publicized faith-community-specific education and information resources

This model challenges a congregation to be innovative in supporting its members who have an advanced illness, and their caregivers, and includes end-of-life and grief issues. Adding a complementary focus of dementia by adopting the Dementia Capable Communities Toolkit is a natural extension of the congregational care work.”

About Our ACT Community

Rather than a geographic action community, the Minnesota Council of Churches (MCC) oversees a faith Action Community, involving churches that committed to creating a dementia-friendly environment for their congregations. The ACT Community was formed in 2014 however MCC no longer has an action team.
