  • Hosted lunch-and-learn sessions

About Our ACT Community

In the Cloquet area, home and community-based providers and medical providers have often worked together to support successful aging as the “Aging Well in Cloquet” group. In 2014, working with the Arrowhead Area Agency on Aging, the group became an ACT on Alzheimer’s ACTion Team. The team had members from skilled nursing facilities, community-based providers, a tribal organization, local government, health care, hospice care, the faith community, and business, as well as a person living with dementia and her caregiver.
